Gabriella many nurses have built their own businesses and are self-employed. Some nurse practitioners own their own practices and provide health care for their own patients. I know nurses who own continuing education companies. They have many employees and provide conferences and online education. I know nurses who are independent consultants on quality and professional development. Some nurses are health coaches or advocates, they might work in IT or informatics. These are all roles in which someone could be self-employed.

That is depending on how you define "medical". For the broad terms, it can be relating to personal health, life style, medical retailing, etc. That is not necessary to be professional in the area.
