I think you may referring to the number of specialties in medicine in which case there are many many specialties. For instance if you want to take care of kids you can be a pediatrician. But if you want to take care of kids, but only want to take care of kids with ear problems you would be a pediatric ENT (ear nose and throat) doctor.
