Most people determine their specialty during their rotations during the third year of medical school. Sometimes people enter medical school knowing they want to do OB/GYN from past experience working in that type of medicine as a scribe or nurse.

Estelle’s Answer
I went to medical school planning to go into pediatrics, but I enjoyed ob/gyn so much more! I decided in third year of med school that I loved the procedures and surgeries. I enjoy the longevity of taking care of patients from their to teen years to the decades beyond. I have had some patients for 25 years and taken care of their daughters as well. I also enjoy the balance of clinic practice and operating days, so every day is interesting.
