talk to the patient to determine their interests and priorities, review the medical record and research the diagnosis to get an idea of potential progress speed and assess the patient's current status. Combine this information to select goals that are achievable.

For you to be successful in the rehabilitation protocol, you have to make a good assessment of the patient. a complete anamnesis. Then, in the light of all that has been collected, set short, medium and long-term goals.

That is something you should determine while completing the initial evaluation, by talking to either the client and/or the family/legal guardians (depending on the client's ability to elaborate their wants/needs). Assessing their current status, then educating the client/caregivers about the purpose of therapy and what they would like to improve/restore/etc. They may not think of goals, so be ready to offer ideas; sometimes you list something and they have no idea that they CAN work on it, or it never occurred to them that they could work on it or improve something.
