From a physician's point of view, what has worked for me is to explain the science behind whatever disease process and how medication or therapy can alleviate it using common terms. Sometimes I even draw little illustrations to get my point across.

Provide explanations using language that the patient understands without being condescending or patronizing. Explain the plan including the expected outcome is and what could go wrong if the plan is not followed.

The health care teams approach and strategy when caring for patients impacts outcomes of health and the future patient/health care team relationship.
Your question on motivation is a very important point to bring up. My experience has been mostly with parents and teenagers, as my experience is in pediatrics. It was difficult at times to motivate and keep the parents motivated to adhere to treatment regimens or life style changes.

Education on the outcomes and reinforcing the rationale of health behaviors and therapy provided has worked for me. Allow for open, two way communication, with the family and patient is important. Evaluating the level of understanding. Being a great listener demonstrates you care and that motivates patients to be the most compliant with your recommendations. Always close your conversations with a future plan for follow up and give options on schedule and ways to contact you for any other concerns.

The patient is the center of the health care team. Motivation begins with the patient/caregivers. In my experience, if the patient or the family lack motivation, it may be very difficult to maintain that motivation as a nurse or physician.
