My office pet peeve is really around "office politics". The idea that you'd have to tip-toe around people and appease certain people's egos is a bit maddening. Unfortunately, it seems to be quite unavoidable, regardless of industry or company. There may be varying degrees of it with some places being more severe than others, or perhaps more isolated instances of the "office politics", but they exist everywhere from what I can see.
Ideally, a workplace is more of a meritocracy and a supportive/collaborative environment, but it's not always the case.

In staffing, I dislike when a hiring manager supplies you with an incomplete job description yet in a phone call they have a lot to share.
When you supply or reveal to candidates a job description or posting that is not convincing it makes it harder for myself to resonate with the hiring manager.
