Build a plan, which have to be based on the knowledge about yourself. You have to be aware of your goals and dreams. Once you have that information:

Setting goals is part of the path to develop yourself. Goals that take years to achieve may seem not relevant on that path but the knowledge gained on that path should help you understand what you should plan for your future.
You may, and in my personal perspective, should evaluate your goals continuously and align then to your reality.
The fact you set a goal for a specific professional target, and learn that your much more interested in a different one as you grow up, you should consider it again. Maybe even decide from the beginning that once every 6 months you'll verify your goals.
I did it myself, set and changed goals as I learned the new goals better suited my career evolvement over the years. I very happy with my decisions.
And specific to your question, a doctor career requires many qualities and resources. I believe that if you do have those qualities, you will be able to balance between that career and your art desire.

Develop a plan to hit your career goals. Determine what things you need to do to become a doctor and research those things. Reach out to career professionals to build a network to gain even more knowledge and learn from their experiences. Compile all of this together to create a plan for your goals.
