I work as a Nurse Case Manager, this is a field that is a bit different than hospital nursing, but with a lot of the similar functions of hospital nursing. I provide education to patients about their medical condition, assist in making certain the patient has everything they need prior to discharge from the hospital. Such as arranging home health care, skilled nursing placement or making sure the patient has the equipment to move around safely in their home environment. I educate the patients on their medications and ensure they have the resources they will need to stay healthy and hopefully avoid another hospital admission.
This is all done over the phone, which may be a bit difficult since one is does not have the ability to assess the patient physically, but it is a very rewarding career to know one is helping a person when they are at their most vulnerable and needing a lot of care and compassion.
I have done hospital nursing and case management is a whole different area but just as rewarding.

The primary role and responsibilities of nurses is to care for patients, community, advocate for the best quality healthcare for all, educate, provide interventions to patients and community, work nationally and/or internationally, and improve healthcare for the present and future.
Commonly, most are aware of nurses working within the hospital setting. However, nurses also work in various settings such as in clinics, schools/colleges/universities, government/politics, businesses, legal settings, internationally, consultants, own their own business, independent clinical practice, movie sets, authors, and so many more that there is not enough space to list them all.
In the settings where nurses work, they are assessing patients’ physical, emotional, psychological, social, and spiritual health and wellness. Nurses identify what are the problems, challenges, and strengths of people and the community. Nurses work in collaboration with other healthcare providers (e.g. providers such as physicians, nurse practitioners, midwives, physician assistants, social workers, nutritionist and more). Nurses provide interventions from administering medications (if the nurse has an advanced degree and approval by a state the nurse can prescribe medications), treatments, referrals to resources, education for health and wellness, and advocacy. Nurses use critical thinking skills to put together information about a patient, community, and situation to determine the safest care, identify when to collaborate with providers/healthcare professionals/others, and intervene rapidly to prevent further harm in a situation.
