The teams who harvest organs, and/or transplants are experts in their field.
Watching the process is extraordinary and seems flawless, these experts are amazing!
The organs are typically placed immediately placed in temperature controlled coolers, and either airlifted out or driven to the location based upon the viability, and donor recipient match.
From what I have been told is that when the person whom organs are taken from, and are placed into the appropriate matched recipient it all takes place simultaneously.

It is so advanced now that a kidney transplant can be administered quite safely. Transplant nephrology is a very established field now.
