In process assurance, you don't work on your own, but in the large accounting firms, you in many ways act as an independent worker marketing yourself to different work teams, building your resume of clients you've worked on, and making a network of people from different cities and departments within the company.

Many dermatologists join small groups and are their own bosses. This is the most common set up for a Derm practice. Starting a practice from scratch is also possible, but it is a lot of work and riskier/expensive.

If you are considering a career as a self-employed physician, I would steer you away from a surgical specialty. We have relatively frequent emergencies such that it is difficult to take call all the time as a solo practitioner. Also, we are in the operating room half of the week, and this means that your office is empty and your staff is not busy.
