I am not a psychologist, I am a therapist with a master's degree. In my profession, the biggest obstacle is knowing that people have to want to get better. Sometimes as therapists, we have a deep desire to help our client's but the client isn't ready to receive help. I work with many individuals who are addicted to substances and they come to therapy but aren't ready to do the hard work of making the necessary changes. Sometimes they have a traumatic past and it takes time to begin to unravel what is keeping them in their addiction. I have learned to be patient and to provide clients with unconditional positive regard acknowledging the struggle they are facing in overcoming their addiction. It can be a slow process. It can be difficult to watch a client struggle but it can also be rewarding to see them move forward and begin to heal and make changes.

I think it depends on what area of psychology you are practicing. Often it's the access to patients. For example, in an outpatient setting, the patients or clients have to show up for their appointments so that you can see them. Giving a reminder call can help with this.
I have also worked in a prison or jail setting. There, you are dependent on the correctional officers to escort the patients to your office. So it helps to have a good relationship with the CO's.
