I would utilize something called a Sports Resume.
They are similiar to job resume's, but instead the emphasis areas include, past education and accomplishments, athletic teams and achievements, sports awards, positions played, and overall athletic skills.
There are examples of Sports Resume's found online, along with sports cover letters.
Upon completing a resume, send it directly, along with a cover letter to the coach of the college that you would like to attend.
Send it through the mail, directly to the coach, instead of utilizing emails, because coaches receive a lot of emails. This will enable the information to land right on his desk. Hope this has been helpful. Best of luck.

If your playing baseball in H.S. work with your coach on film and season stats. Put together a product that you can send to schools of your choice and forward to the athletic department.
