When it comes to planning ahead for establishing a career for yourself, I think the first thing to do is establish your priorities. Ask yourself "What do I really want?" and take the time to answer that honestly. I'm talking about what you want out of life and what you want from yourself. What matters most to you?
If the answer doesn't come easily, that's great! Spend as much time as it takes searching your soul, learning about yourself and the world around you. And try some things. Pay attention to how you feel as you pursue different things. You don't have to know all the answers, and you don't have to be extremely sure. But you need at least an idea - a direction.
Once you have that, you can begin to make plans for how to create a life (and your career will be a big part of that life) that will allow you to experience what matters most. To do that, you need to investigate what types of things lead to what you want. Ask questions of those around you that have been what you've been through, and that have what you want. Find out what they did. Learn about how they made decisions and plans.
You'll find that you want some things more than others. So prioritize those things. Always focus first and foremost on your top priority, and do what you can to be moving toward that.
Other than that, I will advise you to remember that nothing ever works out as planned :) Stuff comes up, life happens. That doesn't mean you shouldn't plan, but instead that you should be flexible. Your career pathway will be a journey. Enjoy that journey for what it is, be flexible, and remember that the journey is really the purpose of it all (whether or not we arrive at our destination is irrelevant).

Consider setting SMART goals - those that are Specific, Measurable, Achievable, Relevant, and Time-Bound. Begin by identifying the results or targets you're aiming for. Be as detailed as you can. If multiple ideas come to mind, feel free to repeat this process as often as you need. This way, you're actively working towards your goals.

I like to write whatever it is I’m planning. Identify clearly what are you planning for. Are you going some place, is it for an event. What are the criteria’s, descritiond, details like time of day, number of attendees etc. I also like to have a mental preview or picture of what you’re planning for, run the scenario step by step in your mind so you can figure what you will need.
