I would like to give you advice, but I can't actually give you any answers because I am not sure which branch of the medical field you have been in or what your degree and certifications are.

If you are a medical doctor and came over on a work visa, many work as Patient Care Technicians, Medical Assistants, Nurses or Lab Technicians. To work as a medical doctor, you'd have to obtain an American Medical degree and Certifications. Many American doctors can practice as doctors overseas, but the American medical industry requires that doctors from other countries get an American medical education. Since you are near San Francisco, I would suggest that you visit some medical schools, one being the University of California, San Francisco Medical School and ask around specific questions at their admissions office.

Since you have not worked in your field for five years, and I do not know which medical job you had, I would suggest picking something that is similar to what you already have your degree in or something that would be a two year program to start anew. You can always check with social service agencies, employment services offices or any group affiliated with the country you were educated and employed in. This is definitely something that you will have to explore, specifically with agencies that specialize in your exact field of medical service and someone who could guide you about the immigration guidelines around employment in your field of work.
